Hypertension and diabetes are among the leading causes of preventable illness and death in Ghana. Left untreated, these chronic conditions can lead to heart attacks, heart failure, stroke, kidney disease, vision loss, and other life-threatening complications.

At Mansa Memorial Hospital, we are committed to reducing the burden of these diseases through evidence-based prevention, early detection, education, and ongoing community support.

Preventing Hypertension and Diabetes Through Community Care

Our Community Health program empowers individuals to take control of their health by promoting healthy lifestyles, improving access to screening, and supporting long term disease management. We provide education, health coaching, and personalised guidance to help patients monitor their blood pressure and blood sugar, adhere to prescribed medications, maintain healthy diets, increase physical activity, and achieve healthy weight management.

Our Community Health Nurses and Programme Coordinators deliver outreach services through individual consultations, community education sessions, health screenings, and support groups across Akyawkrom, Asotwe, Besease, Yaw Nkrumah, Atia, Ejisu, and surrounding communities. 

By bringing healthcare directly into the communities we serve, we are improving access to care, reducing health disparities, and helping people live longer, healthier lives.
